SHIFT Transition to Adulthood Conference
SHIFT is a free interactive training for families and youth with disabilities ages 14-21 who are navigating the transition to adulthood.
All local and state COVID protocols will be followed.
Join us as we explore person-centered planning and learn about the transition process, Vocational Rehabilitation services, Independent Living Centers, Medicaid waivers and so much more. Families and youth will leave with a plan outlining next steps for their transition from high school to adult life.
You will leave with tips, resources, and information to help navigate:
- The Road to the Good Life. Explore ways to use person-centered planning as part of the Individualized Education Program (IEP). Gain a better understanding of how to use the transition to adulthood process to prepare students for life after high school.
- The Road to Careers and Employment. Learn how Vocational Rehabilitation and Pre-Employment Transition Services can assist youth in gaining and maintaining employment.
- The Road to Independence and Interdependence. Gain information for accessing supports and services that will help the youth in their communities.
