SHIFT Transition to Adulthood

Parents and youth with disabilities, age 14-21, are invited to join us.

We will explore person-centered planning and learn about transition services in the IEP, Vocational Rehabilitation, Independent Living Centers, Medicaid waivers and more. Families and youth will leave with a plan outlining next steps for their transition from high school to adult life.

Training will start promptly at 9:45 a.m.

  • The Road to Independence and Interdependence

Parents and youth with disabilities, age 14-21, are invited to participate in this free interactive training to

  • learn about transition as part of the Individualized Education Program (IEP).
  • discover how Vocational Rehabilitation (VR) and Centers for Independent Living (CILs) can help.
  • hear stories of people with disabilities who are living their good lives.
  • get information for accessing supports and services.
  • gain information regarding the Home and Community Based Services (HCBS) Medicaid waivers.
  • leave with next steps for adult life, career planning and job support.
